Effective 1 July 1996, the method of recording CEUs will change. The new method will be much easier on everyone, but it will require that credit be claimed at the meeting for both pre-approved and post-approved CEUs.

For meetings with pre-approved CEUs, the person sponsoring the meeting will provide a sign-in sheet. You will have to print your name, social security number, and certificate number on the sheet. There will be a separate number and sign in sheet for each 1/2 day meeting. For meetings with concurrent sessions, there will be a separate number and sign-in sheet for each session. If you don't sign in at the meeting, you will not receive credit. The person sponsoring the meeting will mail the CEU sheets to Madison.

For meetings without pre-approved CEUs, one of you will have to get the person responsible for the meeting (or just do it yourself) to make up a sign-in sheet. On the sign-in sheet you will need names, social security numbers and certificate numbers. At the top of the sheet, write in a brief meeting name, date and time. You will need a separate sheet for each 1/2 day meeting. Mail the sign-in sheet(s) and a copy of the program to me. A copy of a proper sign-in sheet can be printed from this internet site by using the print option on your browser (Sign-in Sheet). Make sure the name, phone number and address of the person responsible for the meeting is on the program. I will get with the person responsible for the meeting, assign CEUs and send the information to Madison. It will be much easier, however, if you insist that the person responsible for the meeting obtain pre-approved numbers.

You still have the option of claiming self-directed CEUs but it is best to use the pre- or post-approved system. You can check the status of your CEUs at anytime by calling 608-273-8085. I will mail CEU summaries about once a quarter so that you can check and make sure you receive proper credit. If you believe that your CEUs have not been properly recorded, your fist attempt to fix the problem should be between you and Madison.
