Completion of the AD-421 Report:
1. Go to web page http://cwf.uvm.edu/cris/intro.htm
2. Under “Enter Forms,”
click on “Accomplishment Reports (AD-421)”
3. Choose State (“
4. Mark small button
labeled “
Password is “sunshine.”
5. Enter investigator name (lead person on any project, grant, etc.)
6. “Project Selection” page displays—this will list all projects with the same lead PI. Choose one by marking the appropriate box.
7. “Enter a Progress or Termination Report” page displays.
Text is entered into boxes—can be cut-and-pasted from other text software.
At bottom of page, click “Check and Save.”
8. “Information entered” page displayed.
This is for a check of information, for both the PI as well as the CRIS system. An automated check is done for text in all boxes.
At bottom of page, click “Go to Screen 2”
Provide information in boxes, as in step 7, above.
Click on “Check and Save.”
If information as displayed in correct, Click “Confirm Report”
This will save all text entry, such that it will be retained. PI’s can leave the web site and make additions/edits at another time.
9. “Confirmation Screen” displays.
PI’s can click ( to "Select Project”) in order to complete reports on other
projects.
10. After “Report Complete” button is clicked, a final screen displays stating “…report … submitted to K. Bowen….” And “You can a final copy.”
