Once completed, the AD-421 form(s) are saved.  After "save report" is entered, and using the 'save as' under the 'File' menu of Netscape, these forms can be saved to personal hard drives, etc., for inclusion in this document.  A suggestion for the file name is "lastname_AD421" with a suffix on this file name (i.e., "_1", "_2") when multiple reports are submitted.

If you are not lead PI on a project requiring completion of an AD-421 form, inclusion of these forms are not required.  You must, however, provide a list of projects by lead PI, AAES number, and title.
