Forms The following forms are available for downloading. Save the file(s) to a disc and view or print using WordPerfect 6/7/8 (.wpd) or Microsoft Word (.doc). These documents are now available in Adobe Acrobat PDF form. Click on the "Get Acrobat Reader" button to download Acrobat Reader to your hard drive. Remember where you loaded the file. After the software is loaded on your computer, you can click on a .pdf file to view it. If your browser asks you what application to use, find the AcroRd32.exe file you just loaded on your hard drive and click OK.
* The "Application for Certification" and the "Application for Certification of Turf Grass" must be completed, signed and all supporting documents submitted by mail. They cannot be faxed. Other forms may be filled out and faxed.
