Blank Card for Printing or Photocopying

Instructions for completing the card:

It is very important that cards are properly filled out each time. If you do not know what to write in a space, it is better to leave it blank than to guess.

The best way to collect information is to have a supply of data cards (make copies from this page or request printed cards from the Natural Heritage Program) with you when in the field. When you observe an animal, fill out the card on the spot.

If you know the USGS quad and/or the township, range, and section of your observation, it will help us map it. If you don’t know, leave it blank. Just be sure to provide detailed directions that will enable us to map the record with reasonable accuracy.
